What information do I need to enrol a patient?
In addition to the prescription information, three unique patient identifiers are required: initials, date of birth and email address. If a patient is under 20, caregiver details are also required.
Care coordinators may choose to include additional information about their patients, as Blue Tree NZ may serve as a repository for patient data in your practice.
Patient initials and date of birth are the only identifiers visible to the Roche CSP Coordinator. The email address is required as a third unique system identifier (but is not visible).
The patient’s or caregiver’s email also enables the patient to receive their CSP enrolment letter automatically and provides the option for the patient to access Blue Tree.
The patient will receive these automatically after enrolment as two emails. It is optional for the patient to access Blue Tree. If they choose to, they will be able to view details about their treatment journey, including their clinician and centre as well as their CSP structure and timing.
